  • Photo ID
  • Recent blood pressure (within 6 months) required for estrogen birth control.
  • Age eligibility:
    • Birth control, emergency contraception, period delay: 18-54
    • Hormone therapy (HRT): 40-65

Birth Control with Estrogen and HRT: Require a recent accurate blood pressure.

❗️No BP? You can obtain one at a pharmacy kiosk, urgent care, fire station, or home BP cuff.

⚠️ ⚠️ Please don’t submit a combined pill (COC) birth control request or a hormone therapy (HRT) visit without an accurate blood pressure. Visits are NOT refundable due to not having accurate blood pressure reading.

Recent mammogram (within last year) is required for hormone replacement therapy with estrogen. 

Birth control visit info

Starting, restarting, or switching birth control methods.

Includes combined pills, progestin-only pills, patch, ring or Phexxi when medically appropriate. 

Age eligibility: 18-54

*BP needed for estrogen containing methods.

Emergency contraception info

Use if you had unprotected sex or contraceptive failure.

Works best the sooner you take it.

Age eligibility: 18-54

Delay your period info

Short-term medication to postpone bleeding for travel or events.

Must start at least 3 days before your expected period.

Age eligibility: 18-54

Hormone replacement therapy (HRT) info

For symptoms related to perimenopause or menopause such as:

  • Hot flashes
  • Night sweats
  • Sleep disruption
  • Vaginal dryness

Age eligibility: 40-65
Hormonal and non hormonal options available 
Flat $75 visit.

* BP and mammogram required for estrogen containing therapy. 

Refills & follow-up visits

If your pharmacy shows refills available, request it directly from the pharmacy.

If you have 0 refills remaining, start a new visit.

Birth control and HRT renewals may require updated blood pressure readings.

Follow-ups may require new visits for symptom review and medication monitoring.
